Nexletol (bempedoic acid) is a once-daily oral tablet that lowers LDL cholesterol by 15โ25%. It is especially useful for patients who cannot tolerate statins due to muscle side effects. The CLEAR Outcomes trial showed it reduces heart attacks and cardiovascular procedures in high-risk statin-intolerant patients.
Nexletol (bempedoic acid) was FDA-approved in February 2020, developed by Esperion Therapeutics. It is the first in a new class of oral LDL-lowering drugs called ACL inhibitors (ATP-citrate lyase inhibitors). Unlike statins, which work in muscle as well as the liver, bempedoic acid is activated only in the liver โ which is why it does not cause the muscle pain (myalgia) that some patients experience with statins.
How Does Nexletol Work?
Bempedoic acid blocks ATP-citrate lyase (ACL), an enzyme in the cholesterol synthesis pathway that sits upstream of HMG-CoA reductase (the target of statins). By inhibiting this earlier step:
- The liver produces less cholesterol
- The liver compensates by upregulating LDL receptors on its surface
- More LDL is cleared from the blood โ LDL falls by 15โ25%
Key difference from statins: Bempedoic acid is a prodrug activated by an enzyme (ACSVL1) present in the liver but not in skeletal muscle. This is why it lowers LDL without causing the muscle side effects that lead many patients to discontinue statins.
Who Is Nexletol For?
- Statin-intolerant patients with established cardiovascular disease or familial hypercholesterolemia who need additional LDL lowering
- Patients already on maximum tolerated statin + ezetimibe who need further LDL reduction but are not candidates for injectable therapies (PCSK9 inhibitors)
- Also available as Nexlizet (bempedoic acid + ezetimibe combined tablet) for patients who need both agents
๐ Statin Intolerance: True statin intolerance (muscle pain or weakness caused by statins, confirmed by rechallenge) affects an estimated 5โ10% of patients. Bempedoic acid fills an important gap for these patients โ an oral, once-daily option that does not cause myopathy.
CLEAR Outcomes Trial โ The Evidence
The CLEAR Outcomes trial (2023) enrolled 13,970 statin-intolerant patients with or at high risk for cardiovascular disease, randomized to bempedoic acid 180 mg daily vs. placebo. After ~40 months:
| Outcome | Bempedoic Acid vs. Placebo |
|---|---|
| LDL reduction | ~21% reduction from baseline |
| Major adverse cardiovascular events (MACE-4) | 13% relative risk reduction |
| Non-fatal heart attack | 23% reduction |
| Coronary revascularization | 19% reduction |
This was the first randomized controlled trial to show that a non-statin oral LDL-lowering drug reduces cardiovascular events in statin-intolerant patients.
Dosing and Administration
- Dose: 180 mg orally once daily
- Can be taken with or without food
- Can be used alone or added to statin (if tolerated) and/or ezetimibe
- Available as Nexlizet (bempedoic acid 180 mg + ezetimibe 10 mg) in a single combination tablet
Side Effects and Cautions
- Gout / hyperuricemia: Bempedoic acid increases uric acid levels by ~1.2 mg/dL on average. This can trigger gout attacks in susceptible patients. Inform your provider if you have a history of gout. Uric acid should be monitored.
- Tendon rupture: Rare reports of tendon rupture, particularly Achilles tendon. Caution in patients with prior tendon disorders.
- Mild increases in creatinine (generally not clinically significant)
- Unlike statins โ does NOT cause myopathy or liver enzyme elevation at meaningful rates
โ ๏ธ Gout Warning: If you have a history of gout, tell your doctor before starting Nexletol. Your uric acid and gout symptoms should be monitored. Some patients may need prophylactic urate-lowering therapy.
Nexletol vs. Other Non-Statin LDL Options
| Drug | LDL Reduction | Route | Key Feature |
|---|---|---|---|
| Nexletol (bempedoic acid) | ~15โ25% | Oral daily | No myopathy; CLEAR Outcomes data |
| Zetia (ezetimibe) | ~15โ20% | Oral daily | Blocks intestinal cholesterol absorption |
| Repatha (evolocumab) | ~59% | Injection Q2W or monthly | Maximal LDL lowering; FOURIER data |
| Leqvio (inclisiran) | ~50% | Injection Q6 months | siRNA; office-administered twice yearly |
Key Takeaways
- Nexletol (bempedoic acid) is a once-daily oral tablet that lowers LDL by ~15โ25%
- Activates only in liver โ does not cause the muscle pain associated with statins
- CLEAR Outcomes trial: 13% reduction in MACE in statin-intolerant high-risk patients
- Can raise uric acid โ caution in patients with a history of gout
- Useful as add-on to maximum tolerated statin/ezetimibe, or as alternative in true statin intolerance
- Also available as Nexlizet (bempedoic acid + ezetimibe combination)
