We accept most commercial insurances.
We generally accept MEDICARE with MEDICAID but we may not be in network with some Medicare Advantage Plans.
We DO NOT accept new MEDICAID or MEDICAID with commercial insurance plans.
HMO plans generally require insurance approval. You will have to pay as selfpay if you see us without authorization.

Your responsibility is determined by YOUR INSURANCE PLAN, we have to follow your insurance plan's decision.
You may have to pay a full amount set by your insurance plan until you meet the deductible. We do our best to estimate the amount but it is only an estimate. The final amount comes after your insurance processes the claim.

We will send you an email invoice after insurance claim processing, please pay the invoice on time. We provide the service under the good faith that balances will be paid. If you pay more than your responsibility, we keep it as credits for future visits but please contact us if you want refund. 

If you join any Medicare Advantage plan, it is extremely important that you give us that information. You will always have your traditional medicare number but you have to use your Medicare Advantage plans for your visit with us.

For straightforward conditions, we have affordable self pay fees $175 for a new visit, $125 for a follow up visit. The very basic set of labs starts from $50 (some labs can be several hundred dollars).  We cannot provide comprehensive services to our selfpay patients because of limited labs and medication availability.
