FreeStyle Libre — Clinic Code: diabetesplus
FreeStyle Libre 2 and Libre 3 users can share their glucose data with our clinic through the LibreView platform.
Steps to Connect:
- Open the LibreLink app on your phone
- Go to Menu → Connected Apps → LibreView
- Select "Add Practice"
- Enter clinic code: diabetesplus
- Confirm — you're connected!
Alternatively, visit LibreView.com, log in, go to My Practices, and enter code diabetesplus.
💡 Tip: Make sure your LibreLink app is set to sync automatically so your readings upload in real time. Our team can review your ambulatory glucose profile (AGP) report before and after your appointments.
Dexcom — Clinic Code: diabetesplus
Dexcom G6 and G7 users can share their data through the Dexcom Clarity platform.
Steps to Connect:
- Go to clarity.dexcom.com and log in with your Dexcom account
- Click Sharing in the top navigation
- Select "Share with a Clinic"
- Enter clinic code: diabetesplus
- Confirm — data will begin sharing automatically
You can also manage sharing through the Dexcom G7 or G6 app under Settings → Share.
What Happens After You Connect?
Once connected, our team can:
- Review your Time in Range (TIR), average glucose, and glucose variability
- Identify patterns — overnight highs, post-meal spikes, pre-meal lows
- Make medication or insulin adjustments between appointments when needed
- Send you a message through your patient portal if we see concerning trends
You remain in full control of your data and can disconnect sharing at any time.
Frequently Asked Questions
Do I need to share before every appointment?
Once you connect, data shares continuously. You don't need to do anything before each visit — your data is always available to our team.
What if I don't have a smartphone?
For Libre users without a smartphone, you can use the Libre reader to scan your sensor and then upload data to LibreView via a computer at home. Contact our office for assistance.
Is my data private?
Yes. LibreView and Dexcom Clarity are HIPAA-compliant platforms. Only authorized members of your care team can access your data.
I use a different CGM — can I still share?
Contact our office regarding other CGM brands (Medtronic Guardian, Abbott Lingo, Stelo, etc.). Data sharing options vary by device and platform.
Can I still share data if I use an insulin pump?
Yes — if your pump is integrated with your CGM (such as Omnipod 5 with Dexcom G6, or Tandem t:slim with Dexcom), the CGM data uploads through the same Clarity or LibreView platforms. Pump-specific data (from Omnipod, Tandem, or Medtronic) may be viewable through their respective clinic platforms — ask our team for details.
