How to Become a New Patient
Scheduling with us is straightforward. Most commercial insurance plans do not require a physician referral, though some HMO plans may. If you are unsure, please call your insurance company before your appointment.
Schedule Your Appointment
Book online through our patient portal or call/text us at 832-968-7003. New patient appointments are available at both Sugar Land and Memorial City.
Complete Your Patient Forms
After booking, you will receive a secure link by email or SMS to complete your intake forms electronically before your visit. This includes your medical history, medication list, and insurance information.
Arrive 15 Minutes Early
Please plan to arrive at least 15 minutes before your scheduled time to complete any remaining check-in steps. Patients who arrive late may need to reschedule.
Meet Your Provider
At your first appointment, your provider will review your history, discuss your concerns, and develop a personalized care plan. Many patients have lab work ordered at or before the visit.
What to Bring to Your Appointment
Having these items ready helps ensure a smooth, productive first visit.
Photo ID
A valid government-issued photo ID such as a driver's license or passport.
Insurance Card(s)
Your primary and secondary insurance cards. We will verify eligibility at check-in.
Medication List
A complete list of all prescriptions, supplements, vitamins, and over-the-counter medications you currently take.
Medical Records
Any relevant records, lab results, or imaging from prior providers — especially recent labs and specialist notes.
Payment Method
Your co-pay or any office balance due. We accept credit cards, HSA/FSA cards, and cash.
CGM or Device Data
If you use a continuous glucose monitor or insulin pump, bring your device or printed reports if available.
Please note: If you arrive without completing your intake forms or more than 15 minutes late, we may need to reschedule your appointment to ensure adequate time for your care. We appreciate your understanding.

Your intake forms are sent electronically before your appointment. Please complete them as soon as possible — this allows your provider to review your history in advance and make the most of your visit time.

Insurance & Self-Pay
We participate with Medicare and most major commercial insurance plans. A referral is generally not required, but patients with HMO plans should verify with their primary care physician beforehand. We do not currently accept Medicaid as a primary insurance.
