What do RPM and CCM mean?
Remote Patient Monitoring (RPM)
A device at home, such as your CGM, a scale or a blood pressure cuff, sends readings to your care team. Problems can be spotted between visits instead of waiting months for your next appointment.
Chronic Care Management (CCM)
Regular check-ins, usually monthly by phone, for people with two or more long-term conditions such as diabetes and high blood pressure. Staff help with your care plan, medicines and questions between visits.
Is there a cost?
Medicare and many insurance plans cover these programs, but a copay or coinsurance may apply depending on your plan. Ask the program or call us before you agree if you have questions about cost.
Do I have to join?
No. These programs are optional extras. You can decline or stop at any time, and you will keep seeing your doctor here as usual.
Questions about a program?
Call or text our office and we'll explain which program your doctor recommended and why.
