Fructosamine to Estimated A1C

Fructosamine reflects average blood glucose over the past 2–3 weeks, making it useful when A1C is unreliable (hemoglobin variants like HbS or HbC, hemolytic anemia, iron deficiency, recent transfusion, or pregnancy). This calculator converts fructosamine to an estimated A1C equivalent.

Nathan formula: A1C (%) = (Fructosamine / 59.3) + 1.61
Cohen formula: A1C (%) = 0.017 × Fructosamine − 0.16

Normal fructosamine: 200–285 μmol/L (non-diabetic)
A1C goals: <7% (most adults with diabetes), <6.5% (tight control), <8% (older/frail adults)
⚕️ Note: Fructosamine-to-A1C conversion formulas are approximations with significant inter-individual variability. They are most useful for trending glycemic control, not for making precise diagnostic thresholds. Serum protein levels (especially albumin) affect fructosamine values — low albumin gives falsely low fructosamine. Always interpret alongside clinical context.
