Step 1: Your Correction Settings
A sliding scale tells you how much rapid-acting insulin to take based on your current blood glucose. It is built from two numbers: your target blood glucose and your insulin sensitivity factor (ISF) โ how many mg/dL your glucose drops per 1 unit of insulin.

Your Sliding Scale
Check your blood glucose, find the row it falls in, and take the correction dose shown using rapid-acting insulin. Wait at least 3โ4 hours between correction doses to avoid insulin stacking.
Doses are rounded to your selected increment. Rows above your maximum dose say "call the clinic" on purpose โ very high readings deserve a phone call, not just more insulin.
How to use this scale: โ Check blood glucose before meals and at bedtime (or as directed). โก If below 70 mg/dL, treat the low first โ do not take correction insulin. โข Find your reading in the table and take the dose shown. โฃ Do not repeat a correction dose within 3โ4 hours. โค If above the top row twice in a row, or if you have nausea/vomiting, call the clinic at 832-968-7003.

This does not account for insulin on board (a recent bolus still working) or carbs you're about to eat. For meal dosing, combine with your insulin-to-carb ratio.

Sliding scale insulin alone is a reactive strategy; most people do better on a proactive basal-bolus plan. It is not appropriate for suspected DKA (blood glucose persistently >300 mg/dL with nausea, vomiting, or ketones) โ that is an urgent medical situation.
